Researchers analysed cough sounds recorded through the Sleep Cycle app by roughly 100,000 users in 32 cities across 12 countries. The dataset covered 500 days, enabling the team to compare night-time coughing patterns with daily pollution changes.

A rise of 10 micrograms of particulate matter per cubic metre of air, measured as μg/m³, was linked with a 2.4% increase in coughing over the following five days. The association persisted after the researchers accounted for factors that can also influence cough levels, including weather and influenza activity.

“This suggests that there may be no such thing as a safe threshold for exposure to air pollution, although our research is purely observational,” said Cecilia Mascolo, who led the research at Cambridge.

“However, people may wish to consider how they might mitigate their own exposure to air pollution, by using masks or air filters, for example.”

The observational nature of the study means it identifies a relationship between pollution and coughing rather than proving that particulate matter alone caused each symptom. Still, the consistency of the pattern across a large, international sample raises concern about the effects of day-to-day pollution changes.

Wildfire smoke showed a sharper effect in Los Angeles

The researchers also assessed Los Angeles during 2025, when the city experienced its worst wildfire season in years. In that setting, an increase of 10 μg/m³ in fine particulate matter was associated with a 10% jump in nocturnal coughing.

That result was substantially larger than the average effect observed across the wider dataset. It suggests that severe pollution episodes, including those linked to wildfire smoke, may rapidly intensify acute respiratory symptoms. For people who already notice coughing, wheezing or throat discomfort when air quality worsens, the findings underline why short-lived pollution events can still be significant.

Fine particulate matter is small enough to be inhaled deeply into the respiratory system. The study focused on the practical symptom of coughing rather than only on pollution readings, highlighting how an air-quality change can become noticeable in everyday life—particularly at night, when a cough can repeatedly break up sleep.

European cities were mostly below the WHO daily guideline

The 13 European cities included in the research had average daily fine-particle concentrations ranging from 5.47 μg/m³ in Uppsala, Sweden, to 14.70 μg/m³ in Bergamo, Italy. Their combined mean was about 10.58 μg/m³.

The World Health Organization recommends keeping 24-hour average exposure below 15 μg/m³, except on no more than three to four days each year. Yet the study found increased coughing even in urban settings where typical local levels sat below that recommended daily exposure figure.

European locations in the sample generally had low-to-moderate baseline pollution compared with some cities elsewhere in the study. Beijing recorded an average of 40.97 μg/m³, while Shanghai reached 33.48 μg/m³. The contrast shows that respiratory effects were not limited to the most heavily polluted environments.

“Everyone coughs from time to time, but coughing, whether it’s a one-off or more persistent, is a sign of irritation or inflammation in the lungs,” said lead author Tong Xia from Tsinghua University.

“A cough could be a sign of something that hasn’t become chronic yet, but might in future.”

The findings add to the case for treating air quality as a near-term health concern, rather than only a long-range environmental issue. Even modest shifts in pollution may affect how comfortably people breathe and sleep, particularly in dense urban areas where exposure can change from one day to the next.
